QMHP- CS I/II Crisis Prevention Specialist
- Completes Follow Ups for Mobile Crisis Outreach Team and Continuity of Care following initial crisis intervention.
- Provides crisis counseling and support as deemed necessary.
- Coordinates referrals to community-based providers.
- Provides on-site crisis assessments and develops intervention plans.
- Provides crisis assessments in the community including hospitals.
- Prepares progress notes and other required documentation.
- Travels locally and regionally.
Requires successful experience in providing mental health services for people with severe and persistent mental illnesses, a working knowledge of psychiatric diagnosis, psychosocial assessments, and crisis intervention.
Once year of case management preferred.
Education:
Graduation from an accredited college or university with a bachelor’s degree or master’s degree, with a major in behavioral, social sciences, humanities, or education. KNOWLEDGE, SKILLS, AND ABILITIES
- Knowledge of mental illness and emotional components of physical illness, psychiatric diagnosis, psychosocial assessments, and crisis intervention.
- Demonstrated skill in interviewing and reporting results of interviews.
- Considerable ability to work cooperatively and effectively with professional personnel (i.e. jail staff, hospitals, shelters, etc.); to exercise tact, initiate, and good judgement in dealing with consumers and families; to prepare reports and make recommendations for administrative use; to speak and write effectively; to relate to people and to interact with consumers.
- Ability to lift up to 30lbs.
- Ability to learn about and implement trauma-informed care practices within the scope of work for the position.
- Must have excellent time management, filing skills, and clinical documentation skills.
- Ability to travel and transport clients 60% locally and 40% regionally.
- Must have and maintain a level of general health that permits the performance of the job duties and ability to maintain regular attendance as set forth in center policies and procedures.
- Ability to assist with daily living skills.
- Must have knowledge of rehabilitation/skills training and treatment planning.
Hours: Mon-Fri 8am-5pm
Salary: $21.6346-$25.4639 Hourly $45,000-$52,965 Annually **Depending on education and experience.
