Location: The Woodlands / Conroe / Willis, TX
Schedule: Monday–Friday (Full Time)
Pay: Extremely Competitive Salary + Mileage Reimbursement

On-Call: Not required! Optional on-call shifts available for additional pay

About the Role
Concho Hearts Hospice is seeking a full-time RN Case Manager to join our compassionate and experienced team. This role is perfect for someone who wants to be more than just a nurse — someone who wants to make a real difference in the lives of patients and families.

What You'll Do

• Manage a personalized caseload of hospice patients
• Provide direct patient care with empathy and excellence
• Complete timely documentation and collaborate with the IDG
• Serve as a trusted partner to families throughout the hospice journey
• Optional: Pick up on-call shifts for additional compensation (not required)

What We're Looking For

• Active RN license in Texas
• Hospice experience is a plus but not required- Skill can be taught pasion cannot.
