Bilingual Patient Educator - Job Overview
Schedule: Monday-Friday (8AM-4:30PM) or (8:30AM-5PM)

Key Responsibilities
- Implement health education programs in individual, co-visit, and group settings to enhance patient knowledge and self-efficacy.
- Assist patients in setting health-related goals based on comprehensive assessments of needs and readiness for change.
- Conduct point-of-care testing such as HIV, and pregnancy tests.
- Guide patients in navigating healthcare systems to achieve their health goals.
- Document all patient interactions in electronic health records and data systems.
- Collaborate as a member of an interdisciplinary clinic team to foster effective relationships.
- Develop and maintain educational materials, tools, and resources for patient and community education.
- Participate in the design and evaluation of new educational curricula and materials.
- Meet performance metrics and contribute to grant/contract requirements as applicable.
- Engage in clinic, team, and departmental meetings, trainings, and workshops.
Minimum Qualifications
- High School Diploma or equivalent required.
- 3 - 5 years of relevant experience.
- Bachelor's Degree in a related field with a minimum of 1 year of relevant experience preferred.
- Experience providing direct education to clients of diverse cultures and backgrounds in one-on-one and/or group settings.
- Experience in a healthcare setting.
- Experience with computers and computer programs.
- Ability to provide point-of-care testing within 3 months if/as required per site assignment.
- Ability to demonstrate core competencies within 3 months of hire.
- Bilingual in English and Spanish.
