Hospice Marketer/Community Liaison
Location: Beaumont and the surrounding area

At Concho Hearts Hospice, our liaisons are trusted partners in the healthcare community. We’re looking for a motivated professional who is passionate about hospice care and enjoys developing meaningful partnerships with providers throughout the Fort Worth area.

What You’ll Do:

• Develop and maintain referral relationships with hospitals, physicians, assisted living communities, and skilled nursing facilities
• Grow referral sources throughout the Fort Worth service area
• Provide hospice education to healthcare providers and community partners
• Meet with patients and families to explain hospice services and obtain consents
• Coordinate closely with the clinical team to ensure seamless admissions
• Represent Concho Hearts Hospice as a trusted community resource

What We’re Looking For:

• Minimum 2 years of hospice marketing experience required
• Proven relationship-building and networking ability
• Self-motivated with strong organizational skills
• Comfortable presenting hospice education to providers and community groups
• Ability to work closely with clinical teams to coordinate patient admissions
