Pharmacist II

Under the direction of the Clinical Coordinator or Manager and the Pharmacy Director; optimizes patient medications therapy by providing comprehensive clinical pharmacy services. Works collaboratively with the health care team to accomplish goals. Full-Time, night shift.

Job Requirements:

• Minimum Education
  • B.S. Pharmacy or Pharm D; or Master of Pharmacy
  • Continuing education as required for licensure by TSBP
• Minimum Work Experience
  • Previous hospital experience with clinical responsibilities
• Required Licenses/Certifications
  • Active registration as a licensed pharmacist in the state of Texas
  • Sterile Compounding Certification
  • BLS; ACLS; or PALS; depending on practice area
  • Registration as a preceptor with TSBP
• Required Skills; Knowledge; and Abilities
  • Assess; document; and perform physician consultations; excluding total parenteral nutrition; but including all medication dosing consults
  • Perform home medication reconciliation
  • Provide education presentations to pharmacy staff and other staff upon request
  • Ability to effectively direct and/or supervise personnel as appropriate
  • Ability to handle confidential information
  • Ability to maintain professional appearance and decorum at all times
  • Ability to demonstrate excellent oral and written communication skills
