- Already have established, active relationships with hospital discharge planners, case managers, hospitalists, or social workers in DFW who have sent you referrals in the past 24 months.
- Willing and able to do daily in-person visits to hospitals, SNFs, and senior living communities (100 % field-based, no remote/hybrid).
- Have your own reliable vehicle and clean driving record.

The Role in One Sentence
Leverage your existing relationships with DFW-area discharge planners, case managers, and referring physicians to drive patient referrals to our high-quality in-home physical, occupational, and speech therapy services.
Who We Need
A proven referral-development professional who is already known and trusted by hospital and SNF staff in the DFW market and can immediately start generating new patient volume.
Must-Have Requirements (Non-Negotiable):
- 2+ years of successful healthcare referral development or liaison experience in the DFW market.
- Current, active relationships with discharge planners, case managers, hospitalists, or social workers at multiple DFW hospitals and/or skilled nursing facilities who have sent you referrals in the past 24 months.
- Live within the DFW metroplex today (relocation and remote candidates will not be considered).
- Comfortable and excited about daily in-person visits (this is not a desk job).
- Reliable vehicle and clean driving record (mileage reimbursed).
Nice-to-Have (Not Required):
- Experience specifically with home health, home therapy, outpatient clinic or Pharmacy referrals.
- Existing book of business you can bring or reactivate.
Compensation:
- Base salary $70,000 – $75,000 (based on strength of existing book/network).
- Uncapped commission – top performers clear $150k–$200k+.
- Mileage reimbursement + full benefits (health, dental, vision, 401k match, PTO).
- $3,000 employee referral bonus for the person who refers the hired candidate.
