Physical Therapist, PT - Home Health - Full-time
We are seeking a dedicated and skilled Physical Therapist for our Home Health division to provide high-quality, patient-centered care in the home setting. This role is essential in fostering a positive rehabilitative environment for patients recovering from illness, injury, or surgeries. As a Physical Therapist in home health, you will evaluate each patient's physical condition, develop individualized treatment plans, and implement therapy interventions aimed at maximizing each patient’s independence and quality of life. You will engage closely with patients, families, and caregivers to educate them about their conditions and the necessary steps for recovery. This position demands effective communication skills, clinical expertise, and a compassionate approach as you work with individuals of varying needs and backgrounds. The successful candidate will possess a strong understanding of physical therapy practices and demonstrate the ability to adapt therapies to suit the home environment. • Evaluate patients through physical assessments and medical history
• Develop personalized treatment plans based on individual needs and goals
• Implement therapeutic exercises and training programs
• Educate patients and their families on injury prevention and wellness
• Monitor and document patients' progress toward rehabilitation goals
• Collaborate with other healthcare providers to ensure holistic patient care
• Maintain accurate patient records and documentation in compliance with regulations
Qualifications:
- Master’s degree in physical therapy
- 1 year of home health experience as a physical therapist
- Currently licensed to practice as a physical therapist within the state of agency operation and compliant with accepted professional standards and practices
- Must possess and maintain valid CPR certification while employed in a clinical role
- Must be a licensed driver who can travel to all business locations
