Physician – Post-Acute Care | Flexible Schedule | Tyler, TX Physician-Led Organization | Flexible Schedule | Competitive Compensation Trumen Physicians + Associates is seeking an experienced and compassionate Physician to join our growing post-acute care team in Tyler, TX. Responsibilities include managing acute and chronic conditions, facilitating care transitions, collaborating with facility staff, and supporting value-based care initiatives. Core Responsibilities: * Perform in-person assessments and manage treatment plans in skilled nursing facilities * Collaborate with interdisciplinary teams to ensure continuity and quality of care * Update documentation and EMR in a timely and accurate manner * Participate in Trumen’s value-based and transitional care programs * Build and maintain strong relationships with facility staff and administration * Be available on-site during scheduled days Qualifications: * MD or DO with a current, unrestricted Texas medical license * Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine * Minimum of 1 year of experience in post-acute, geriatric, or SNF settings preferred * Strong clinical judgment, communication, and organizational skills * Reliable transportation within the Tyler area Why Live in Tyler, TX?
