Case Manager, Perks and Benefits Starting Day One
Encompass Health Corporation is seeking a dedicated Case Manager to join our dynamic team in providing exceptional care and oversight to patients across our healthcare services continuum. As an integral component of our patient care approach, the Case Manager will collaborate with healthcare professionals to deliver seamless and comprehensive services suited to the individual needs of each patient. You will play a pivotal role in assessing patient needs, planning and coordinating treatment plans, and ensuring the highest quality of health care services, tracking patient progress and adjusting treatment plans as necessary. This position is paramount in bridging effective communication between healthcare providers and patients to ensure that patient care goals are met and exceeded.
