Advance Practice Provider (NP /PA) - Primary Care

Overview:

Full-time, Monday - Friday, no weekends or eves. Manage chronic conditions of high-risk geriatric patients in their homes across North Houston with a supportive team (MA, RN Care Coord, Social Workers, Schedulers) and a company car.

Responsibilities:

The Advance Practice Provider (APP) we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The APP who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications:

• Licensed as a NP or PA in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
