Resident Specialist YHDP HOTTCHY

The Resident Specialist YHDP plays a critical role in supporting the health and stability of transitional age youth (18-24) experiencing homelessness and residing in transitional housing under the Youth Homelessness-Demonstration Program (YHDP). Provides basic healthcare support, health education and assistance with medical coordination while working closely with homeless services to promote overall wellness, housing stability and independent living. The main goal of the Resident Specialist YHDP is to provide support while in transitional housing for transitional age youth in order to prevent future homelessness.

EXAMPLES OF WORK PERFORMED:
  • Assist Transitional Age Youth residents with basic health-related needs including hygiene, wound care, medication reminders, and chronic condition monitoring (e.g. diabetes, asthma, etc.).
  • Assure transitional housing environment is clean and assist clinical staff with inventory, ordering, re-stocking, and organization of household.
  • Conduct regular bed checks and curfew, wellness checks, and document changes in residents’ physical and emotional health.
  • Collaborate with clinical and case management staff to develop individualized care and wellness plans.
  • Deliver skilled mentoring and coaching in symptom management, personal hygiene, nutrition, food preparation, exercise and community integration.
  • Coordinate the distribution of information between department staff and other community agencies including returning phone calls, faxing documents, relaying messages, and receiving and making copies.
  • Provide supervision and assistance with school assignments when indicated.
  • Educate residents on personal health, nutrition, and preventive care in a friendly and culturally competent manner.
  • Provide transportation to transitional age youth receiving AMH services, or in support of education, employment and housing opportunities, from one location to another. Transportation services may also be provided to/from community outings or as need arises. Travel is 15% locally.
  • Consistently and accurately write clinical and administrative documentation as directed.
  • Adhere to regulatory requirements and follow all safety, medical and administrative procedures and protocols. Resolve conflicts or report incidents to supervisor.
  • Monitor and document self-administration of medication, determine the effectiveness of medication, identify side effects of medications, and emphasize the importance of taking medication as prescribed.
  • Provide reimbursable paraprofessional services to include skilled mentoring and coaching, paraprofessional aid, job placement support, and permanent housing opportunities.
Education: High school diploma/GED or equivalent.

Experience and Training:
  • Must have current valid driver’s license and current and continued proof of liability insurance.
  • Experience working with the homeless population is preferred
KNOWLEDGE, SKILLS AND ABILITIES:
  • Ability to provide resident supervision.
  • Knowledge of understanding basic medical care, patient hygiene, and chronic disease management.
  • Familiarity with the unique health and social challenges facing transitional age youth experiencing knowledge of trauma-informed care principles, harm reduction, mental health first aid.
  • This includes understanding, recognizing, and responding to the effects of trauma, and an ability to work in a behavioral environment.
  • Good work habits and organizational skills.
  • A bility to document progress notes.
  • Attend work in a timely manner and on a regular basis to ensure adequate staffing and provision of services.
  • Demonstrate courtesy and respect in relationships with others within the organization and external to the organization.
  • Does not engage in frequent conflicts with others.
  • Communicate effectively, keeping supervisor adequately informed with accurate and complete information, and willingly and effectively perform assigned duties.
  • The person in this position will serve as a role model for all transitional age youth in regard to appearance, interaction, problem solving methodology, personal accountability and demonstration of respect for others, self and property.
  • Ability to learn about and implement trauma-informed care practices within the scope of work for the position.
  • Level of general health permitting stooping, bending, climbing stairs and participating in moderately active recreational activities.
  • Applicants must be able to successfully participate, perform, and complete the skills required for CPR, First Aid, and PMAB certification.
Hours : Mon-Fri 3pm-12am

Salary: Hourly $15.00 Annual $31,200
