Job Summary:

The Behavioral Health Crisis and Prevention Coordinator position is a challenging and rewarding role that supports our clients and providers when they need it most. It is an exciting, but demanding opportunity to grow with our Whole Person Care (WPC) team as we care for our clients in crisis.

The Behavioral Health Crisis and Prevention Coordinator supports CHN patient services, including patient service specialists, communications center associates, care coordination, eligibility, medical providers, medical assistants, dental providers, dental assistants and other

to schedule immediate provider visit or mental health deputy for high-risk clients. This role supports the implementation of the Suicide Prevention and Management standard of care. The individual will provide the highest quality of customer service care and use de-escalation techniques during time-sensitive and sensitive interactions with patients, patient's families,

community partners, and fellow coworkers whether on phone, via chat or email, or in person, to meet the needs of our patients and the goals of CHN. Other duties in support of this main focus may include welfare checks, supporting immediate incident response, and other support of high-risk populations as assigned by management.

This role also provides care coordination services on identified patient populations to include medication, appointment, and treatment adherence. The patient populations may be diagnosis-based or program-based.

The care coordination activities will be tracked and benchmarked against outcomes goals with performance activities to enhance service goals. The quality outcomes are aligned with Certified Community Behavioral Health Clinic (CCBHC) programs and additional quality programs (TJC, ACO, HEDIS). port of high-risk populations as assigned by management.

This position requires the ability to manage multiple priorities and well as clear, concise and consistent communication with stakeholders. The behavioral health crisis and prevention coordinator will demonstrate through their work ethic to possess a patient-centered focus, collaborative team player, sense of optimism, ownership and commitment to their position.

Duties and Responsibilities:

  • To provide real-time crisis scheduling support
  • To perform evidence-based care coordination to support prevention of hospitalizations by promotion of treatment plan adherence and CHN whole person care services, such as provider consults, technology access, and wraparound services
  • Promotion of group therapy including recruitment and supporting access to transportation services
  • Real-time crisis scheduling support according to CHN standards of care ·
  • Coordinates safety checks on clients as requested by provider and/or management: welfare checks, contacting local police, mental health deputy, crisis mobile unit, emergency contacts, etc.
  • Care Coordination: Evidence-based outcomes with goal of preventing hospitalization, incarceration, and harm reduction
  • Completes incident reports per CHN policy and standards of care
  • Ensures accurate documentation in care coordination consult entries
  • Adheres to applicable CHN policies and procedures within scope of work. Promotes professional and positive demeanor in all interactions with patients, providers, coworkers, and management.
  • Communications with patients are accurately documented. Messages routed to correct recipient
  • Assists to resolve patient concerns by using problem-solving skills based on training provided. Appropriate escalation to management for higher level of assistance
  • Maintains a working knowledge of the OpenFQ [EHR system] to properly manage behavioral health provider schedules. Completes prior and day of confirmations, reviews accounts to provide patient with all pertinent information prior to their appointment
  • Reviews new and established patient charts to ensure all documents [i.e. ID, consent, and demographic] forms are present and up-to-date. Review with the patient of all the information which is being collected on the registration forms [i.e. patient portal, permissions, and etc.]
  • Prepares to discuss with patient any co-payments, co-insurance, and/or deductibles which need to be collected prior to patient being seen
  • Reconciles cash drawer with OpenEMR reports.
  • Ensures cash and credit card receipts are prepared and ready for transfer to the accounting department by end of shift
  • Adheres to CHN policies when releasing protected health information [PHI] throughout all interactions with patients, patient's families, community partners, and fellow co-workers

Required Skills/Abilities:

  • Familiarity with office equipment: computer keyboarding, calculators, photocopiers, and faxes
  • Ability to communicate clearly and effectively
  • Ability to work with people of all socioeconomic levels
  • Strong team player with a focus on collaboration
  • Ability to respond effectively to the most sensitive inquiries or complaints
  • Ability to handle multiple projects simultaneously
  • Consumer advocate for quality health care and services

Preferred Skills/Abilities:

  • Bilingual ability in Spanish
  • Knowledge of risk assessments and crisis management
  • Basic knowledge of suicidal/homicidal ideation warning signs and risk indicators

Required Education and Experience:

  • High school diploma or GED
  • 1-2 years experience in direct patient communications

Preferred Education and Experience:

  • 2-3 years experience in behavioral or mental health
  • Experience with de-escalation techniques
