• Location: Northampton, MA 01060 (Massachusetts hospice jobs)
• Assignment Type: Locum tenens / Travel Registered Nurse
• Healthcare Setting: Hospice
• Start Date: August 25, 2025
• End Date: November 23, 2025
• Contract Length: 13 weeks
• Schedule: Monday – Friday, 8:00 AM – 5:00 PM
• Shift: 8-hour day shifts, 40 hours per week
• Estimated Weekly Pay: $1970–$2020 per week
• EMR System: HCHB (Homecare Homebase) experience required

Registered Nurse Qualifications

• Active Registered Nurse (RN) license in Massachusetts
• Minimum 12 months of recent hospice nursing experience (within the past 3 years)
• Proficiency with HCHB (Homecare Homebase) EMR system
• Excellent communication and organizational skills
• Ability to work independently and collaboratively within an interdisciplinary healthcare team

RN Case Manager Responsibilities

• Provide compassionate case management and clinical support for hospice patients and their families
• Develop, coordinate, and implement individualized patient care plans
• Collaborate with interdisciplinary team members to ensure high-quality patient outcomes
• Accurately document all nursing care activities in the HCHB EMR system
• Educate patients and families on hospice care, symptom management, and available healthcare resources
• Advocate for patient comfort, dignity, and quality of life throughout the end-of-life journey
