• Conduct comprehensive in-home patient assessments
• Develop and update individualized care plans
• Manage medications, wound care, and chronic disease support
• Educate patients and families on self-care and health management
• Coordinate care with physicians and interdisciplinary teams
• Supervise and support LPNs and Home Health Aides
• Ensure accurate, timely documentation of patient progress
• Monitor and adjust care based on patient condition changes

• Active Registered Nurse (RN) license
• Prefer a minimum of 2 years of nursing experience (Home Health preferred)
• Strong clinical judgment and assessment skills
• Ability to work independently in the field
• Excellent communication and patient education skills
• Valid driver’s license, reliable transportation, and auto insurance
• Current CPR certification
• Experience with HomeCare HomeBase preferred (not required)

This is not a hospital floor role. This is relationship-based nursing , where your work is personalized, meaningful, and seen directly in patient outcomes. You’ll have the freedom of field nursing with the support of a collaborative clinical team behind you.
