Licensed Vocational Nurse (LVN), IDD Services
Location: South Austin, TX
Hourly Rate: $27
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Your Role
As the LVN in our IDD program, you will play a critical role in supporting the health and well-being of the individuals we serve. This position blends clinical care, coordination, documentation, and field-based support.
The LVN facilitates weekly on-site clinics with our visiting primary care physician and ensures continuity of care across our group home locations. Responsibilities include conducting medication check-ins, completing face-to-face nursing assessments, and monitoring health conditions to ensure quality services are provided.
You will be responsible for documenting all services provided, with an expectation to meet billable service requirements for individuals served. The LVN is primarily based out of our South Austin D&S office, spending approximately 60% of time in-office and 40% in the field. Travel to group homes, day programs, and other work sites is required using your personal vehicle (mileage reimbursed).
Additional responsibilities include:
Administer medications, provide documentation, and oversee staff who administer medications
Provide prescribed medical treatment per physician orders and personal care services
Conduct assessments and implement healthcare services within each Individual Service Plan (ISP)
Respond appropriately to changes in condition
Provide training to staff on universal precautions, infection control, and medication administration
Serve as liaison and advocate with medical providers and external agencies to ensure coordinated, consistent care
Qualifications
Associate’s Degree in Nursing or related field, or state certificate
One year of nursing experience working with the population served
Current state LPN/LVN licensure
Valid driver’s license, car registration, and auto insurance
Required certifications per state regulations (CPR/First Aid, HIV/AIDS certification, BBP, ACT training, etc.)
Strong communication skills with staff, individuals served, and families
