We are currently seeking a licensed Physical Therapist (PT) to provide in-home physical therapy services for adult and geriatric patients in West Houston, Katy, and Sugar Land. The PT will conduct evaluations, develop treatment plans, and deliver skilled therapy designed to improve mobility, function, and independence for patients recovering at home.

This is a flexible PRN or part-time opportunity, ideal for PTs looking for autonomy, supplemental income, or a balanced caseload with a trusted, locally managed staffing agency.

• Perform in-home PT evaluations to assess patient strength, balance, and functional mobility
  
  

• Develop and implement customized treatment plans to achieve patient-centered goals
  
  

• Provide hands-on therapy, education on exercise programs, and home safety modifications
  
  

• Accurately document all care provided in compliance with payer and agency requirements
  
  

• Collaborate with the interdisciplinary care team, including OTs, nurses, and caregivers
  
  

• Travel to patient homes across the designated service area
  
  

• Active Texas Physical Therapist (PT) license
  
  

• Minimum of 1 year of experience as a physical therapist (home health preferred)
  
  

• Current CPR certification
  
  

• Reliable transportation and valid driver s license
  
  

• Excellent communication skills and clinical judgment
  
  

• Familiarity with EMR systems and Medicare documentation standards preferred
