ESSENTIAL FUNCTIONS
- Care Coordination
- Act as the primary contact for oral oncolytic patients, ensuring seamless transitions across care settings.
- Oversee baseline and monitoring lab tests, diagnostic procedures, and adherence to treatment plans.
- Coordinate Advanced Practice Provider (APP) education sessions and expand availability across locations.
- Patient Advocacy and Education
- Provide individualized education on disease specifics, treatment plans, and medication adherence.
- Support patients and families with emotional, psychosocial, and logistical challenges during treatment.
- Quality Assurance and Risk Mitigation
- Ensure timely capture of patient consents to reduce legal and quality accreditation risks.
- Monitor compliance with institutional and national care standards, identifying gaps and implementing improvements.
- Adherence Monitoring
- Track patient progress and medication adherence, providing interventions when necessary.
- Collaborate with OCP and other specialty pharmacies and internal teams to maintain continuity of care.
- Interdisciplinary Collaboration
- Partner with care teams to address barriers to treatment, coordinate referrals, and optimize workflows.
- Communicate with home health, hospice, and other external providers to align care plans.
- Documentation
- Maintain accurate records of patient encounters, care plans, interventions, and outcomes in the electronic medical record system.
- Professional accountability is expected of each professional licensed nurse, under the Nurse Practice Act of Texas and policies and procedures of the practice:
- Knowledge of laws governing the practice of nursing in Texas
- Knowledge and implementation of accreditation guidelines
- Identification as LVN via ID badge during all patient contact
- Current licensure by the State Board of Nurse Examiners
- Reported to administration with each renewal period
- Administrative Role includes indirect nursing activities that promote quality cancer patient care within the practice. This includes, but is not limited to:
- Collaboration with physicians and Ohe ffice Team to t identify patient care flow issues and collaboratively identify process changes when indicated.
- Development of education programs. This includes ongoing evaluation and modification of content, tools, and methods.
- Time management of clinic periods, both physician and nursing
- Collaborative staff meetings for upgrading / revision of clinical services and nursing practice
- Cost effective time and equipment/supply management
- Provide adequate coverage of duties when out
- Work collaboratively with clinic support staff to maintain patient workflow throughout the day
- Provide clinical information required by the clerical staff to obtain authorization for care, and optimize patient insurance resources
- Coordinate care with physician and pharmacy staff members
- Report to Nursing Manager and / or Practice Administrator
Additional Functions:
- Program Development
- Assist in developing and updating education programs for patients and providers.
- Support the implementation of workflows to improve consent capture and adherence monitoring.
- Navigation Service Management
- Contribute to principal care management, principal illness navigation, and transitional care projects.
- Coordinate with existing navigators to balance w orkloads and optimize care delivery.
- Community Engagement
- Represent the organization in community events, promoting cancer care and education initiatives.
- Participate in developing best practices for oral oncology care in the broader medical community.
- Professional Development
- Stay current with oncology nursing and accreditation standards and integrate new knowledge into practice.
- Collaborate with leadership to refine processes and support institutional goals.
5. Documentation of Care:
o Document care according to the practice’s policies and procedures. The documentation includes all direct patient care encounters and telephone contacts via the practice electronic medical record system and patient population management technology.

10. Possesses good computer skills and exhibits appropriate phone-etiquette
11. Organizes, plans, and prioritizes responsibilities well
- Unencumbered, current state license to practice as LVN in Texas with good standing
- Oral oncolytic management, prior authorization with PBM and/or case management experience preferred
