Job Description:

• Provide skilled nursing care to patients in their homes
• Administer medications, treatments, and wound care per physician’s orders
• Monitor patient health status and document care accurately using EMR (WellSky/Kinnser)
• Educate patients and families on home care plans and health management
• Collaborate with the care team to achieve positive patient outcomes

Requirements:

• Active Texas LVN License
• Home Health Experience preferred (training available)
• EMR Experience – WellSky/Kinnser a plus
• Current CPR Certification
• Reliable Transportation, Valid Driver’s License, and Auto Insurance
