Join Legacy Community Health as a Maternal and Child Health (MCH) Program Specialist, where you'll play an instrumental role in shaping the future of public health. Stationed at Legacy Montrose Allen Parkway and delivering transformative services across three vital clinics in the Houston area, your work will be at the heart of our mission.
- Address crucial public health issues such as pre-term birth, low birth weight, and maternal and infant morbidity.
- Engage in prenatal and postpartum education and navigation, amplifying community healthcare impact.
- Champion child injury prevention initiatives to safeguard our community's youngest members.
- Enjoy a collaborative and supportive team environment, fostering growth and innovation.
Key Responsibilities
- Serve as a Peripartum-Pediatric Navigator, supporting high-risk pregnant patients with prenatal and post-partum care appointments.
- Facilitate Legacy's prenatal education program through patient scheduling, baby care item distribution, and comprehensive monthly reporting.
- Assist in teaching classes and organizing special events like Baby Shower Parades to enrich community engagement.
- Support Legacy's car seat safety program, including scheduling, inventory management, distribution, and reporting.
- Teach child passenger safety classes and coordinate special events focused on community education.
- Identify and develop educational materials and resources to effectively communicate prenatal, parenting, and child injury prevention information.
- Expand support to other prenatal and parenting programs, including breastfeeding, family planning, and safe sleep, as training and time permit.
- Connect patients with eligibility services and other Legacy primary and preventative health care services.
Minimum Qualifications
- High School Diploma or equivalent.
