JOB SUMMARY
The Wellness Advocate Team Lead is responsible for day-to-day management and oversight of Wellness Advocate performance, providing administrative support, ensuring quality of service delivery, facilitating professional development in support of CHW certification requirements and guiding progress towards intervention goals.
This position is also responsible for ensuring the wellness advocate team completes assigned outreach that is tied to the wellness and prevention program. In addition to programming assistance (i.e., care and resource navigation), this individual will facilitate the execution of CHW strategy within the organization. This includes keeping CHWs up to date on changing requirements related to the eight core competencies of CHW work, enhancing community health initiatives, improving health outcomes, and bridging gaps between members and healthcare providers.
The lead will ensure that CHWs are effectively implementing program interventions, adhering to best practices, and meeting the varied healthcare needs of the populations they serve. They will aid in member support through the development of health promotion and education strategies which includes the importance of screenings, annual checkups, and self-management of chronic health conditions. They will also collaborate with other internal departments to identify and deliver positive change and business efficiencies, highlight operation risks and areas for improvement, and monitor productivity.

JOB SPECIFICATIONS AND CORE COMPETENCIES
The Wellness Advocate Team Lead provides day-to-day oversight and management of CHWs that work remotely with members over the phone. Responsible for a variety of outreach, education, and community coordination activities. Responsible for quality assurance, coaching, and developing team members in all areas of performance, including but not limited to, metrics and behaviors related to the CHWs.
Responsible for managing, planning, and organizing the daily and monthly functions of the team. Ensure team documentation of community work and issue weekly and monthly reports in a timely manner.
Facilitate training and work with other community health workers internally and externally to share best practices and strengthen education and outreach.
Complete ad hoc analysis to support internal and external customers.
Other duties as assigned.

Reports to Position Title: Wellness Advocate Manager

QUALIFICATIONS:
Education/Specialized Training/Licensure: High School diploma or GED equivalent required.

Certification/training as a Community Health Worker preferred.

Work Experience (Years and Area): Two (2) to three (3) years' work experience with patient navigation, care management, or any kind of direct service provision to patients in a community health/outreach, social resource agencies required.

Management Experience (Years and Area): Two (2) years of experience as a lead, supervisor role or equivalent experience required.

Software Proficiencies: Must be computer literate Microsoft Word, Excel and Outlook.

Other: Strong analytical, organizational, prioritization and time management skills. Ability to work independently under minimal direction. Moderate to advanced computer knowledge required
