Community Care Specialist

Requirements
Duties & Responsibilities
- Receive incoming calls and messages for potential clients and consistently strive for emotional linkage and developing relationships
- Determine client needs and assist in coordinating care by referring to appropriate service lines
- Collect, share, and manage information using respective databases
- Clearly communicate detailed information to appropriate teams
- Track referrals, keep status and notes up-to-date in database, and advocate for patient care
- Internally build relationships for the furthering of serving clients
- Make outgoing calls to patients in the care continuum
- Performs other related duties as directed by Supervisor
Qualifications, skills and abilities
- Patient, attentive, and empathic listener
- Must have a strong phone etiquette with the ability to communicate well; both verbally and in writing
- Must be detailed-oriented, accurate, organized, multi-tasker, and great with time-management
- Initiative-driven individual able to work independently and in a group setting in a fast-paced environment
- Computer proficiency and knowledge of Microsoft products
- Must be reliable, self-starter, strong work ethic and work well within a team
- Previous sales or marketing preferred
- Experience in healthcare preferred
- Will train the right candidate
