Physical Therapist - Travel Contract
Physical Therapist — Outpatient | Crosby, TX
Join a growing outpatient therapy team in Crosby, TX. Provide 60-minute evaluations and treatment while developing and implementing individualized treatment plans. Manage patient care under physician direction, document thoroughly in the medical record, and participate in student supervision and professional development activities. Maintain compliance with state practice acts and organizational policies. Demonstrate commitment to patient and employee safety and the organization’s core values.
Minimum Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Texas state physical therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Experience providing outpatient physical therapy including evaluation, plan of care development, and documentation such as letters of medical necessity and functional assessments.
- Ability to perform 60-minute evaluations and treatment sessions.
- Familiarity with clinical documentation and charge entry processes compliant with state and federal guidelines.
- Willingness to participate in student supervision and professional development activities.
Preferred Qualifications
- Experience in an outpatient clinic setting serving adults with musculoskeletal or post-operative conditions.
- Prior experience working within a performance improvement framework and participating in clinic initiatives.
