Home Health RN Job Details

• Position Type: Travel RN Contract
• Location: Pomona, California 91768
• Healthcare Setting: Home Health Nursing
• Start Date: September 29, 2025
• End Date: December 28, 2025
• Assignment Duration: 13 weeks
• Shift: Days, Monday–Friday, 8:00 AM – 5:00 PM (8-hour shifts)
• Hours per Week: 40
• Estimated Weekly Salary: $2200–$2250
• Patient Case Load: Geriatric patients; up to 6 home visits per day (30 productivity points standard)
• Travel Radius: Occasional maximum of 50+ miles; clients grouped to minimize drive time

Registered Nurse Qualifications

• Active California Registered Nurse (RN) license
• At least 1 year of recent skilled Home Health RN experience (within the last 3 years)
• OASIS documentation experience required for start of care
• BLS certification (American Heart Association preferred)
• Reliable transportation for travel nursing assignments

Home Health RN Responsibilities

• Deliver skilled nursing care to geriatric patients in their homes throughout Pomona and surrounding areas
• Conduct comprehensive patient assessments and complete OASIS documentation
• Develop, implement, and update individualized care plans for home health patients
• Educate patients and families on disease management, medications, and home safety best practices
• Collaborate with interdisciplinary healthcare team members to coordinate patient care
• Maintain accurate, timely, and compliant clinical documentation
• Travel efficiently to patient homes within assigned territory, optimizing scheduling and minimizing windshield time
