QMHP-CS I/II DC (Case Worker)
- Provides crisis coverage during scheduled hours for referrals to Crisis Care
- Provides on-site crisis assessments and develops intervention plans
- Provides crisis counseling and supports as deemed necessary
- Coordinates referrals to community-based providers
- Provides continuity of care services with all area providers
- Prepares progress notes and other required documentation.
- Requires successful experience in providing mental health services for persons with severe and persistent mental illnesses, a working knowledge of psychiatric diagnosis, psychosocial assessments and crisis intervention.
- One year case management preferred.
Graduation from an accredited college or university with a bachelor's or master's Degree with a major in behavioral, social sciences, humanities, or education. KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge of mental illness and emotional components of physical illness; psychiatric diagnosis, psychosocial assessments and crisis intervention.
- Demonstrated skill in interviewing and reporting results of interviews.
- Considerable ability to work cooperatively and effectively with professional personnel (i.e. jail staff, etc).
- Must exercise tact, initiate, and good judgment in dealing with clients and relatives.
- Must be able to prepare reports and make recommendations for administrative use.
- Must have ability to speak and write effectively.
- Ability to relate to people and to interact with consumers.
- Ability to lift up to 50lbs.
- Must have excellent time management and organizational/filing skills.
- Must have knowledge and ability to do clinical documentation.
- Must have and maintain a level of general health that permits the performance of job duties.
