Physical Therapist - Travel Contract

Travel Physical Therapist — Home Health — Corpus Christi, TX

Provide in-home physical therapy services to adults and geriatric patients across the Corpus Christi service area. Deliver skilled evaluations, plans of care, and discharge services in compliance with payer and regulatory requirements. Coordinate care with interdisciplinary teams, document visits in electronic health records, and meet productivity expectations set by the client. Travel between patient homes within assigned coverage areas; mileage between first and last patient visits is reimbursed.

Minimum Qualifications

• Master‘s or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Texas Physical Therapy license required to start the assignment.
• Minimum experience: Home Health physical therapy experience required.
• BLS certification from AHA or ARC may be required.
• Experience performing Start of Care (SOC), evaluations, and discharges in a home health setting.
• Proficiency with OASIS documentation required.
• Experience with HCHB electronic health record is preferred.
• Ability to travel within assigned coverage areas including Corpus Christi, Portland, Taft, Ingleside, Aransas Pass, Rockport, Sinton, Odem, Beeville, Mathis, and Robstown.
• Professional demeanor and dependable work history.

Preferred Qualifications

• Prior experience in home health with demonstrated productivity meeting agency expectations.
• Familiarity with home health billing and regulatory compliance.

Assignment Details

• Shift: Days.
• Weekly hour guarantee: 32 hours.
• Productivity expectation: 30 points.
• Coverage area includes Corpus Christi, Portland, Taft, Ingleside, Aransas Pass, Rockport, Sinton, Odem, Beeville, Mathis, and Robstown.
• On-call/weekend requirement: preferred 1 weekend every 6 weeks.
• Mileage reimbursed between visits from first patient to last patient.
