Bilingual Care Team Assistant - Job Overview
Schedule: 8AM-5PM

Key Responsibilities
- Patient Communication: Work with onsite nursing or follow protocol-based triage to address patient phone calls and route clinical inquiries to providers. Return calls and communicate results as directed.
- Pre-Visit Planning: Review patient schedules daily, ensure lab results and consultation reports are available, and gather necessary information. Assist in arranging interpretation services and participate in daily huddles.
- Care Coordination: Manage transitions of care, support access to ancillary services, and maintain the Care Coordination Desktop.
- Medication Management: Assist with medication refills and prior-authorizations.
- Forms: Initiate and complete forms for review and final signature by the provider. Address urgent requests promptly.
- Referrals: Complete referrals, deliver clinical summaries for consultants, and ensure relevant documents are included in patient records.
- Safety and Compliance: Promote effective working relations to facilitate goal achievement and adhere to safety training and equipment operation.
Minimum Qualifications
- High-school level education or equivalency
- Medical Assistant Certification or its equivalent, or at least 2 years clinical experience in an inpatient or outpatient setting
- Must maintain BLS /CPR
- At least 2 years of experience in an ambulatory clinical setting preferred
- Spanish fluency
- Demonstrated competency with an EHRS system
- Excellent communication skills.
- Excellent customer service skills.
- Ability to multi task in a fast paced environment.
- Above average organizational skills.
- Strong typing and computer skills.
- This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management
- Must be able to manage multiple deadline and decision making responsibilities.
