Home Health Licensed Vocational Nurse (LVN)
A Home Health Licensed Vocational Nurse (LVN) is needed in Houston, TX.
Salary and Benefits
- Salary range: $55,000 – $60,000
- Health insurance ($250 reimbursement per pay period)

Responsibilities
- Monday–Friday schedule
- Patient census: 20+ and growing
- EMR: Kinnser / WellSky
- Manage a caseload of home health patients
- Complete and coordinate documentation and patient assessments (within LVN scope)
- Assist with admissions, plan of care coordination, and patient care procedures
- Collaborate with interdisciplinary team members to ensure quality outcomes
- Provide direct nursing care and case management support to patients and families
Requirements
- Clear and active Texas LVN license
- Medicare home health experience
- Familiarity with OASIS documentation
- Strong clinical and organizational skills
