Job Overview
We are seeking a dedicated and compassionate Case Manager to join our healthcare team. The ideal candidate will have experience in hospice care demonstrating a strong commitment to patient service and effective case management. This role is essential in coordinating patient care, ensuring that individuals receive the appropriate services and support throughout their hospital stay and beyond.

Duties

• Conduct comprehensive assessments of patients' needs, including medical, psychological, and social factors.
• Develop and implement individualized care plans in collaboration with healthcare professionals, patients, and families.
• Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
• Facilitate communication between patients, families, and the healthcare team to promote understanding of treatment options and resources.
• Coordinate discharge planning to ensure a smooth transition from hospital to home or other care settings.
• Maintain accurate documentation of case management activities and patient interactions in compliance with hospital policies.
• Provide education to patients and families regarding available resources, services, and support systems.
• Advocate for patients' needs within the healthcare system to ensure they receive appropriate care.

Qualifications

• Bachelor’s degree in Nursing or a related field; RN licensure preferred.
• Experience in case management within a hospital medicine or acute care environment is highly desirable.
• Familiarity with vital signs monitoring and patient assessment techniques.
• Strong interpersonal skills with the ability to work collaboratively as part of a multidisciplinary team.
• Excellent organizational skills with attention to detail in documentation and reporting.
• Ability to manage multiple cases effectively while prioritizing patient needs.
• Knowledge of community resources available for patients post-discharge is an advantage.
• Commitment to providing high-quality patient care with empathy and professionalism.
