Client Care Coordinator

Responsibilities
- Serve as the primary point of contact for clients, providing compassionate support and guidance throughout their senior living journey.
- Assess client needs and help customize service plans that align with individual family circumstances and goals.
- Coordinate communication between families, healthcare providers, and senior living communities to facilitate seamless transitions and ongoing care.
- Maintain detailed and confidential client records, ensuring accuracy and timely updates.
- Educate clients and families on available senior care options, resources, and industry best practices.
- Respond promptly to client inquiries and resolve any concerns with empathy and professionalism.
- Collaborate with the Platinum Senior Services team to continuously improve client experience and service delivery.
- Track and report on client progress and outcomes to support organizational goals and client satisfaction.
Requirements
- Proven experience in client care coordination, social work, healthcare, or a related field.
- Strong interpersonal and communication skills with the ability to provide compassionate support.
- Excellent organizational skills and attention to detail for effective case management.
- Ability to handle sensitive and confidential information with the utmost discretion and professionalism.
- Familiarity with senior living options and elder care resources preferred.
- Demonstrated problem-solving skills and the ability to manage multiple cases simultaneously.
- Empathy, patience, and a passion for helping seniors and their families.
- Proficiency in using computer software for documentation and communication purposes.
