QMHP-CS I/II Case Manager
- Ensure the health records of consumers on assigned caseload are up to date and that necessary records are transferred to the appropriate provider or agency when a referral is made.
- Provides a variety of care coordination activities that ensure consumers on assigned caseload receive the services necessary to achieve the best health outcomes. These activities include but are not limited to; follow up on referrals, navigating local health system, contacting MCO’s to access needed services, assist with arranging transportation to scheduled appointments, assist with transitions between levels of care or care systems.
- Responsible for the coordination of all services which assists clients in their recovery from mental illness and/or substance use including re-engagement after missed appointments and follow-up services post hospital discharge.
- Provides monitoring and tracking to enhance continuity of care from and to external providers, i.e. hospitals, emergency rooms and jails.
- Provides monitoring and follow-up of Individuals identified as at risk of suicide through the care for Life Program. This would include meeting face-to-face, telephone calls and caring contacts as appropriate.
- Updates ANSA and Recovery Plan as appropriate to ensure proper level of service is provided to consumer.
- Performs Related Work as Required.
- Must have Bachelor’s Degree in Psychology, Social Work, or clinically related field.
- Must have current valid TX driver’s license and current and continued proof of liability insurance.
- One year prior case management experience a plus.
- Knowledge of emotional problems and treatment approaches.
- Ability to establish and maintain positive working relationships with clients and colleagues.
- Good work habits and organizational skills.
- Familiarity with Microsoft 365.
- Must have level of general health to permit stooping, bending, climbing stairs and participating in moderately active recreational activities.
- Must have level of agility to permit getting in and out of vehicles on regular basis as well as moving freely to and from vehicle and facilities.
- Physical ability to perform CPR and PMAB skills including lifting up to 30 lbs.
- Must have and maintain a level of general health which permits the performance of job duties.
- Ability to travel 5% locally.
- Must have excellent verbal/written communication skills.
- Knowledge and ability to do clinical documentation.
- Ability to carry out assessments and diagnostic evaluations.
