Job Summary and Qualifications

As a Registered Nurse for Discharge, the top priority is delivering exceptional patient-centered care. They are dedicated to ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings. Specialized assessment skills help ensure that patients are placed in the appropriate care areas and discharged. We offer education and post-hospitalization care as necessary. Their main focus is advocating for patients, families, and caregivers while embodying the organization's vision, mission, and values. The goal is to give an unmatched patient experience and attain optimal clinical outcomes. 

What will you do in this role:



  • The Registered Nurse in Discharge coordinates and delivers high quality, patient-centered care by overseeing the effective management of patients into and out of the acute care hospital setting. The RN Discharge utilizes specialized assessment skills to ensure quality admission to the appropriate care areas, and also safe discharges of acute care patients requiring education and post hospitalization care.

    • Receives physician orders for direct admissions and organizes admission process for patient

    • Obtains a nursing assessment history and ensures proper inpatient care unit admission. Takes appropriate additional action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect

    • Teaches patients/families/caregivers about any non-medicinal follow-up measures, such as healthy diet and exercise, disease prevention, and/or other lifestyle changes. Prepares patients and families/caregivers for future self-management

    • Completes additional discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge

    Job Requirements

    Experience

    • 1+ years of clinical nursing experience as a Registered Nurse (RN)

    Education

    • Associate’s degree in Nursing (ADN) or higher

    Licenses and Certifications

    • Current State of Texas Registered Nurse (RN) license or Compact Registered Nursing License. If Compact License, 60 days to apply for Texas RN license; 6 months to obtain TX RN license

    • Basic Life Support (BLS) certification per St. David’s Healthcare policy

    • Advanced Cardiovascular Life Support (ACLS) certification per St. David’s Healthcare policy – Required within 6 months of hire

    • NIH Stroke Certification within 30 days of hire

St. David’s Medical Center, part of St. David’s HealthCare, is a comprehensive medical facility with three locations: St. David’s Medical Center, Heart Hospital of Austin, and St. David’s Georgetown Hospital.
