Home Health Physical Therapist PT - Full-time
We are seeking a dedicated and experienced Physical Therapist (PT) to join our dynamic team of healthcare professionals providing exceptional home health services in Hamilton. This full-time position offers the opportunity to work independently and make a significant impact on patients' lives by helping them regain their independence and improve their quality of life.
Key Responsibilities:
- Conduct comprehensive patient assessments to develop personalized care plans.
- Provide therapeutic treatments and interventions in the patient's home environment.
- Collaborate with patients, families, and healthcare teams to achieve optimal outcomes.
- Document patient progress and modify treatment plans as necessary.
- Educate patients and their families on exercises and strategies to enhance mobility and function.
- Ensure compliance with all regulatory and professional standards of care.
Qualifications:
- Valid state license as a Physical Therapist.
- Proven experience in a home health setting is preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and manage a flexible schedule.
- Excellent organizational skills and attention to detail.
- CPR certification is required.
- Valid driver’s license and reliable transportation.
