About the Role:

As a Transitional Care Pharmacist, you’ll be the critical link between inpatient and outpatient care. You’ll:

• Conduct thorough medication reconciliation at admission.
• Monitor patient progress and collaborate on discharge planning.
• Coordinate with outpatient pharmacies to ensure seamless medication access.
• Educate patients on their medication regimen before discharge.
• Work across various hospital departments and specialty clinics.

Qualifications:

• Education : BS in Pharmacy or PharmD.
• Licensure : Active pharmacist license in the state of Texas (or ability to obtain).
• Experience : New graduates welcome!.
• Skills : Strong oral and written communication, ability to handle confidential information with discretion, professional demeanor and collaborative spirit, and leadership and supervisory potential.
