Social Worker

Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

• VA Careers - Social Work: Total Rewards of a Allied Health Professional Duties include but are not limited to: The incumbent is responsible for providing case management for a caseload of Veterans in the HUD-VASH program
• The incumbent is responsible for reviewing agreements between Section 8 - landlords - Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements
• The incumbent coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery - and linking and referring Veterans to the VA Medical Center - VA Regional Offices and/or community agencies
• The incumbent complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the HUD-VASH Case Manager
• The incumbent actively participates in program planning and development - advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences
• The incumbent works with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD-VASH Program Veterans
• The incumbent will conduct psychosocial assessments to determine needs of Veterans and family members
• The incumbent will participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate
• The incumbent will address Advance Directives and Organ Donation in accordance with standards of their work area
• The incumbent will coordinate treatment and communicate with the care lines to facilitate continuity of care for Veterans
• The incumbent will maintain data and statistical compilations to comply with JC - CARF - VA and medical center policy and procedures
• The incumbent will assist Veterans and their significant others with coping and dealing with the loss and grief experiences in disability - terminal illness and death
• The incumbent must have a good driving record and be approved and able to drive GOV vehicle for home visits and community visits
• Work Schedule: Monday - Friday
