Description
American Traveler is hiring an experienced RN for a Hospice Case Manager role in a home health setting, requiring HCHB/Oasis experience and admission competency with on-call availability.
Details
- Home health and hospice setting serving patients across a wide geographic territory in central Texas
- Caseload of approximately 4–6 patients per day, with a target of 30 points per week (equivalent to 40 hrs/week)
- Patient population is primarily geriatric, receiving in-home hospice and palliative care
- Cases include in-home patient care, interdisciplinary care coordination, and plan of care development and oversight
- Charting is completed using HCHB (Homecare Homebase) and OASIS; all documentation must be completed at the patient's home
- Schedule is Monday–Friday, 8:00 AM–5:00 PM (5x8-hour shifts)
- Weekend availability required as needed
- On-call rotation required, including approximately one night of call per week (details discussed at interview)
- Admissions are required as part of this role
- Travelers must be willing to travel up to 50 miles from the home base location
- 13-week contract
Requirements
- Active TX RN license or compact RN license currently in hand — no pending licenses accepted
- Current BLS certification required
- Minimum 1 year of hospice nursing experience required; minimum 2 years of overall clinical experience required
- Proficiency with HCHB (Homecare Homebase) and OASIS required; 1 year of experience with both preferred
- Must be comfortable performing same-day patient admissions
- Must be available for on-call rotation, including overnight call shifts
- Must be willing to travel up to 50 miles from the home base
- 2 professional references from a supervisor within the past 2 years required for consideration
- Full 7-year work history with all gaps explained required for consideration
Additional Information
- Responsible for administering skilled nursing care to hospice patients in their homes across diverse environments
- Coordinates care with patients, families, certifying providers, and the interdisciplinary team
- Performs comprehensive patient assessments and collaborates with providers to develop and oversee individualized plans of care
- A tablet will be assigned for charting; all documentation must be completed at the patient's home
- Non-smokers preferred, as the patient population includes individuals with COPD who are sensitive to cigarette smoke
- Scrubs in any color are acceptable
- First-time travelers are welcome to apply
- Local candidates within a 50-mile radius are accepted
