Physical Therapist - Travel Contract

Minimum Requirements

• Master‘s or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Bachelor’s acceptable if equivalent credentials exist.
• Active Texas Physical Therapist license required to start the assignment.
• Current PT professional certification required.
• Home health (HH) and home care/hospice billing (HCHB) experience required.
• Experience assessing and completing plans of care for home health, including SOCs, ROCs, and recertifications/reassessments.
• Experience supervising or collaborating with PTAs.
• BLS certification from AHA or ARC may be required.
• Preferred: orthopedics and neuro rehabilitation experience.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Days 0800-1600.
• Shift Schedule: Monday–Friday, 8:00 AM–4:30 PM (client schedule notes indicate Mon–Fri, 8 AM–4:30 PM and productivity expectations of 30 points).
• Weekly Hours: 40 (weekly hour guarantee noted by client).
• Coverage Areas: Hawkins, Mineola, Lindale; mileage reimbursed between visits from first to last patient.
• Primary responsibilities: conduct home health assessments, establish and document POC, perform SOCs/ROCs/recerts/reassessments, and supervise PTA staff.
