Care Management Coordintor

Are you passionate about helping others navigate their healthcare journey? As a Care Management Coordinator, you will serve as a vital link between members, healthcare providers, and internal teams. This is a telephonic, administrative role focused on ensuring members receive the resources, education, and support they need to improve their health outcomes.

You will be the "first line of support," handling inbound and outbound outreach to resolve issues, schedule services, and provide high-quality customer service while maintaining meticulous records in accordance with state and regulatory guidelines.

Responsibilities

• Conduct telephonic outreach to members to assist with care plan next steps, schedule appointments, and provide ongoing education.
• Connect members to health plan and community resources, specifically addressing Social Determinants of Health (SDOH) needs.
• Manage member onboarding by sending welcome letters, program materials, and related correspondence.
• Act as a point of contact for members and providers to explain care plan procedures, protocols, and resolve inquiries.
• Maintain accurate, non-clinical member records to ensure compliance with state and regulatory standards.
• Work closely with providers to share necessary documentation and facilitate a successful member-provider relationship.
