Care Management Coordintor

work hours: 8 AM - 5 PM
education: High School Responsibilities
- Conduct telephonic outreach to members to assist with care plan next steps, schedule appointments, and provide ongoing education.
- Connect members to health plan and community resources, specifically addressing Social Determinants of Health (SDOH) needs.
- Manage member onboarding by sending welcome letters, program materials, and related correspondence.
- Act as a point of contact for members and providers to explain care plan procedures, protocols, and resolve inquiries.
- Maintain accurate, non-clinical member records to ensure compliance with state and regulatory standards.
- Work closely with providers to share necessary documentation and facilitate a successful member-provider relationship.
