Social Worker
- Assess the social, emotional, and psychological needs of our residents, developing personalized care plans to address their unique needs
- Collaborate with interdisciplinary teams, including physicians, therapists, and nurses, to ensure comprehensive care and support
- Provide counseling, guidance, and support to residents and their families, addressing concerns and promoting emotional well-being
- Facilitate access to community resources, services, and benefits, connecting residents with necessary support networks
- Document all interactions, observations, and interventions in a timely and accurate manner
- Participate in facility-wide initiatives, supporting quality improvement and patient satisfaction efforts
- Bachelor's degree in Social Work or related field (or equivalent experience)
- 0 years of experience in a healthcare setting (or related field)
- Strong communication, counseling, and interpersonal skills
- Ability to work effectively in a fast-paced, dynamic environment
