About the Role

Are you a compassionate and detail-oriented Licensed Practical Nurse (LPN) with a passion for helping patients manage chronic conditions? We are seeking a Remote Chronic Care Manager to support primary care providers in delivering patient-centered, team-based care. In this role, you will work closely with healthcare teams, patients, and caregivers to improve patient outcomes and enhance communication throughout the care continuum.

Key Responsibilities

Support primary care providers in managing a panel of chronic care patients.

Utilize care management platforms to coordinate and track patient progress.

Identify unmet patient needs and facilitate timely interventions.

Engage patients in their own care and encourage proactive health management.

Maintain and manage patient registries to ensure accurate and organized health data.

Collaborate with healthcare teams to enhance care coordination and improve patient outcomes.

Contribute to quality improvement initiatives and population health strategies.

Provide courteous and professional patient interactions via phone and digital platforms.

Ensure compliance with all federal, state, and local healthcare policies and procedures.

Attend and participate in team meetings and training sessions as needed.

Requirements

Active LPN license with at least 1.5 to 12 years of experience (experience in population health is a plus).

Strong problem-solving skills with the ability to multitask.

Excellent written and verbal communication skills.

Strong customer service and interpersonal skills.

Experience with Electronic Health Records (EHR)/Electronic Medical Records (EMR) systems is preferred.

Bilingual in English and Spanish is a plus but not required.

Additional Information

Each Care Manager will be expected to manage a caseload of 500 patients, with 300 billable cases by the end of the month.

This is a fully remote position, but candidates must be available to work during Eastern Standard Time (EST) hours.

The salary for this position is fixed at $22 per hour (non-negotiable).
