Case Manager I, YHDP Drop-In

Youth Homelessness Demonstration Program Case Managers for Klaras Center for Families are responsible for linking and connecting young adults in need of homelessness support programming throughout the six-county catchment area with a focus on those young adults served at the Dobey Drop-In Center.

EXAMPLE OF WORK PERFORMED

• Works with young adults (18-25) and their families in mental health, home, community and/or office settings.
• Provides case management, skills training and crisis intervention for young adults.
• Provides and coordinates life skills training to YHDP Dobey Drop-In Center consumers and works closely with the Navigation Team and other YHDP providers.
• Coordinates Drop-In Center programming, routinely working to secure appropriate community partners with the goal of providing a robust array of homelessness support services for young adults.
• Connects to the Heart of Texas Homeless Coalition’s Heart to Home Coordinated Entry system while also performing other necessary Coordinated Entry functions.
• Complete all documentation in an accurate and timely manner as shown in management reports.
• Refers families of consumers to appropriate social welfare and health agencies to obtain assistance as needed.
• Performs other related duties as required.

Experience:
Must have current valid Texas driver's license and current and continued proof of liability insurance.
Education:
A bachelor's degree (CWI) or master’s degree (CWII) in psychology, social work or related field.

KNOWLEDGE, SKILLS, AND ABILITIES

• Knowledge of and experience working with homeless young adults.
• Ability to connect youth to appropriate community resources as well as establish and maintain working relationships with community partners.
• Knowledge of emotional problems and treatment approaches as well as rehabilitation/skills training.
• Ability to establish and maintain positive working relationships with consumers and colleagues.
• Good work habits and organizational skills as well as excellent verbal/written communication skills.
• Familiarity with computer operation.
• Ability to learn about and implement trauma-informed care practices within the scope of work for the position.
• Must have excellent clinical documentation skills.
• Ability to lift up to 30lbs.
• Ability to travel locally 5% of the time and transport clients.
• Must have and maintain a level of general health that permits the performance of the job duties and ability to maintain regular attendance as set forth in Center policies and procedures.
• Level of general health permitting stooping, bending, climbing stairs and participating in moderately active recreational activities.
• Physical ability to perform CPR and PMAB skills.

Hours: Mon-Fri 8am-5pm

Salary: $45,000-$52,965 Annually $21.6346-$25.4639 Hourly **Depending on education and experience**
