Case Manager I, YHDP Drop-In
- Works with young adults (18-25) and their families in mental health, home, community and/or office settings.
- Provides case management, skills training and crisis intervention for young adults.
- Provides and coordinates life skills training to YHDP Dobey Drop-In Center consumers and works closely with the Navigation Team and other YHDP providers.
- Coordinates Drop-In Center programming, routinely working to secure appropriate community partners with the goal of providing a robust array of homelessness support services for young adults.
- Connects to the Heart of Texas Homeless Coalition’s Heart to Home Coordinated Entry system while also performing other necessary Coordinated Entry functions.
- Complete all documentation in an accurate and timely manner as shown in management reports.
- Refers families of consumers to appropriate social welfare and health agencies to obtain assistance as needed.
- Performs other related duties as required.
Must have current valid Texas driver's license and current and continued proof of liability insurance.
Education:
A bachelor's degree (CWI) or master’s degree (CWII) in psychology, social work or related field. KNOWLEDGE, SKILLS, AND ABILITIES
- Knowledge of and experience working with homeless young adults.
- Ability to connect youth to appropriate community resources as well as establish and maintain working relationships with community partners.
- Knowledge of emotional problems and treatment approaches as well as rehabilitation/skills training.
- Ability to establish and maintain positive working relationships with consumers and colleagues.
- Good work habits and organizational skills as well as excellent verbal/written communication skills.
- Familiarity with computer operation.
- Ability to learn about and implement trauma-informed care practices within the scope of work for the position.
- Must have excellent clinical documentation skills.
- Ability to lift up to 30lbs.
- Ability to travel locally 5% of the time and transport clients.
- Must have and maintain a level of general health that permits the performance of the job duties and ability to maintain regular attendance as set forth in Center policies and procedures.
- Level of general health permitting stooping, bending, climbing stairs and participating in moderately active recreational activities.
- Physical ability to perform CPR and PMAB skills.
