Physical Therapist Job Details

• Location: Agawam, MA (serving Hamden, Hampshire, and Franklin counties; up to 75-mile travel radius)
• Assignment Type: Locum Tenens / Travel Physical Therapist Contract
• Duration: 13 weeks (August 18, 2025 – November 17, 2025)
• Schedule: Monday – Friday, 8:00 AM – 5:00 PM (5x8-hour day shifts)
• Hours per Week: 40 (productivity-based; no guaranteed hours)
• Estimated Weekly Salary: $2050–$2100
• Weekend Rotation: Every 6th weekend
• Mileage: Reimbursed at IRS standard rate (travel between patient homes)
• EMR: HomeCare HomeBase

Physical Therapist Qualifications

• Active Physical Therapist license in Massachusetts (required at time of application)
• Minimum 1 year of recent Adult Home Health Physical Therapy experience (no first-time travelers)
• Current CPR/BLS certification
• Experience with HomeCare HomeBase EMR preferred (or willingness to retrain)
• Reliable vehicle, valid driver’s license, and required liability insurance
• Ability to travel up to 75 miles from Agawam clinic
• Meet productivity expectations (30 points/week, approx. 5–10 patients/day)
• Strong organizational and documentation skills (documentation due within 24 hours of visit)

Physical Therapist Responsibilities

• Deliver high-quality, compassionate physical therapy services to adult patients in their homes
• Conduct Start of Care assessments and follow-up home health visits
• Develop and implement individualized care plans to improve patient mobility and independence
• Travel between patient homes and attend office meetings at the Agawam, MA clinic
• Complete accurate, timely documentation using HomeCare HomeBase EMR
• Collaborate with interdisciplinary healthcare team members for comprehensive patient care
• Participate in weekend rotation (every 6th weekend) as scheduled
• Maintain compliance with agency policies and all regulatory requirements
