Home Health RN Job Details

• Position Type: Locum Tenens (Travel Nursing Contract)
• Specialty: Home Health Registered Nurse
• Location: Houston, TX 77058 (serving Houston and surrounding communities: Katy, Pleak, Galveston, Devers, Anahuac)
• Duration: 8 weeks (Start: 9/15/2025 – End: 11/9/2025)
• Schedule: Monday – Friday, 8:00 AM – 5:00 PM
• Guaranteed Hours: 40 hours per week
• Shift: 8-hour days
• On-call/Weekend: No on-call or weekend shifts
• Productivity: 30 points per week
• Estimated Weekly Salary: $2050–$2100

Registered Nurse Qualifications

• Active Texas Registered Nurse (RN) license
• At least 1 year of recent home health RN experience
• Proficient with OASIS documentation (required for home health nursing jobs)
• Experience with HCHB EMR preferred
• Strong communication and organizational skills
• Ability to travel within the assigned Houston, TX territory

Home Health RN Responsibilities

• Deliver skilled nursing care to patients in their homes throughout the Houston area
• Perform comprehensive patient assessments and create individualized care plans
• Accurately complete OASIS documentation and maintain compliance
• Collaborate with interdisciplinary healthcare team members
• Educate patients and families on disease management and self-care techniques
• Meet productivity standards and ensure high-quality patient outcomes
