Bloom Healthcare is launching its San Antonio market , and we are seeking a mission-driven Physician who is excited to help establish, grow, and shape our presence in this community. This is a unique opportunity to step into a physician role , where your clinical expertise, leadership, and insight directly influence how care is delivered locally.
If you’re looking to move away from rushed visits, RVU pressure, and fragmented care—and toward meaningful patient relationships, manageable volumes, and real work-life balance—this role was built for you.

What You’ll Do:

• Deliver comprehensive, relationship-based primary care through in-home and senior community visits
• Serve as a Bloom ambassador in the San Antonio market, helping establish and grow our local footprint
• Build and manage a patient panel with an average of ~10 visits per day
• Collaborate closely with interdisciplinary teams including specialists, pharmacists, social workers, and care coordinators
• Develop trusted partnerships with assisted living communities, hospitals, clinics, and community organizations
• Use population health data and condition-specific insights to drive excellent outcomes
• Participate in ongoing education and professional development
• Travel occasionally to our Denver market for onboarding and continued training

Schedule & Lifestyle

• Monday–Friday schedule (3, 4, or 5 days available)
• Flexible scheduling with rotating call only 2–3 times per year
• Predictable days and manageable patient volume
• Mileage supported with a monthly auto allowance

Compensation & Benefits

• Competitive base salary: $225,000 – $260,000 , commensurate with experience
• Performance-based bonus potential

Qualifications

• Active and unrestricted Texas medical license
• Passion for caring for medically complex, homebound, and senior populations
• Interest in contributing to the growth of a new market while delivering exceptional care
