MDS Coordinator
Full-Time | MDS/RAI Coordinator (RN or LPN)
Schedule: Monday- Friday
Pay Range: $39.20-53.20/ hour (based on experience) *Same Day Pay Available*

As our MDS Coordinator, you will lead the RAI/MDS assessment process to support quality resident care, regulatory compliance, and accurate reimbursement. Working collaboratively with nursing and interdisciplinary teams, you'll ensure timely assessments, individualized care planning, precise documentation, and continuous improvement in resident outcomes and satisfaction. -Coordinate and complete the full Resident Assessment Instrument (RAI)/MDS process for assigned residents
-Ensure assessments are completed timely and accurately in compliance with State and Federal regulations
-Conduct resident interviews, participate in care conferences, and attend discharge meetings as needed
-Apply knowledge of Case Mix, PPS, Medicare, HMOs, managed care, and third-party payors to support accurate reimbursement
-Utilize ICD-9/ICD-10 coding to ensure accurate documentation and care planning
-Review charts, admission information, and clinical documentation; identify concerns and escalate appropriately
-Partner with nursing and interdisciplinary teams to identify and address quality of care opportunities
-Support the development of individualized care plans based on resident assessments
-Contribute to continuous improvement efforts focused on regulatory readiness, quality outcomes, and resident satisfaction
-Maintain accurate records and reporting using Microsoft Office, EHR systems, ROX, and Excel
-Effectively prioritize workload to meet regulatory and organizational timelines
-Support consistent workflows and documentation standards across the clinical team
Minimum Qualifications
-Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing with the Minnesota Board of Nursing
-Minimum of three (3) years of experience in long-term care
-Demonstrated experience with MDS/RAI processes and Case Mix
-Experience working with PPS, Medicare, HMOs, managed care, and third-party payors
-Experience with care planning in a long-term care setting
-Strong understanding of clinical assessment processes, documentation standards, and regulatory requirements
-Proficiency with Microsoft Office, EHR systems, ROX, and Excel
-Strong written and verbal communication skills
-Ability to work both independently and collaboratively within interdisciplinary teams
-Excellent organizational, time management, and prioritization skills
-High level of professionalism, attention to detail, and commitment to quality care
Preferred Qualifications
-Certification in Gerontology
-Previous experience supporting quality improvement or regulatory readiness initiatives
