Community Health Worker
Summary
The Community Health Worker (CHW) will be working in the Division of Pediatrics-Public Health on upSTART Community Programs. upSTART is a suite of community-based programs designed to support pregnant people and families with young children. This is a grant funded position. The CHW will work with upSTART staff and participants to connect families to needed services within the community, and to help recruit families into our programs.
Job Duties
- Supports staff and participants from upSTART (estimated to serve 1500/year). Specific job responsibilities include:
- Assists front-line staff with identification and connection to resources for participating families.
- Develops relationships with community organizations and facilitate a warm hand off to families.
- Identifies and connects with community organizations for infant and maternal care supplies.
- Develops and maintains a resource list for common goods /services needed in the grant service areas.
- Promotes upSTART programs at community events.
- Collects program data and enters the data into the program and state databases.
- Coordinates with the project staff to schedule visits, send reminders, and contacts participants for follow-up as needed.
- Helps participants connect with resources in their area and assists with applications for eligible programs.
- Documents all activities, service plans, and results.
- Maintains confidentiality per HIPAA guidelines in regards to patient information.
- Attends program staff meetings.
- Attends Division meetings.
- Represents the Division and the upSTART at community events.
- Performs other duties as assigned.
Minimum Qualifications
- High School diploma or GED.
- Two years of relevant experience in health/social services.
Preferred Qualifications
- Bilingual (English/Spanish).
- Community Health Worker certification.
- Bachelor's degree in Social Work.
- Understanding of community resources.
- Experience working with underserved populations.
Work Authorization Requirement:
Candidates must be legally authorized to work in the United States at the time of application and throughout the duration of employment. This position is not eligible for visa sponsorship.
