This travel assignment places you in a long-term care setting, delivering physical therapy services to residents with varied mobility and functional goals. The role is a temporary assignment with a defined 13-week contract and guaranteed hours, beginning in mid-summer.

Responsibilities
- Conduct initial therapy evaluations and develop individualized plans of care for residents requiring physical therapy services.
- Implement evidence-based interventions to improve mobility, strength, balance, gait, and activities of daily living.
- Document patient progress, communicate with supervising clinicians, and coordinate with the care team to adjust goals as needed.
- Monitor patient response and safety, modify treatments for optimal outcomes, and ensure proper use of equipment.
- Adhere to infection control, safety, and documentation policies; maintain accurate records in the patient chart.
- Collaborate with residents, families, and facility staff to support discharge planning, referrals, and transitions of care.

Requirements
- Active Physical Therapist license in Texas or eligibility to obtain licensure
- PT degree (DPT or equivalent) required
- Experience in long-term care or home health settings preferred
- Ability to work in a travel assignment within a day shift framework
- Demonstrated ability to document and communicate effectively with the care team
- Must be able to operate within a facility-based setting and meet assignment requirements
