About the Opportunity

We are seeking a full-time Bilingual Family Nurse Practitioner to support Pair Teams Texas Medicaid Care Program and deliver relationship-centered comprehensive care to patients with complex medical social and behavioral needs.

You will work within a multidisciplinary team of community health workers and nurses to provide continuous preventive chronic and acute care support to Medicaid members across Texas. Many of our patients struggle with chronic illness behavioral health needs SUD or housing instability and benefit greatly from consistent proactive tech-enabled medical care.

While this role is primarily remote candidates must reside in Travis County (Greater Austin TX) for occasional patient visits and periodic in-person care team meetings.

Key Responsibilities:

  • Deliver virtual primary and preventive care to underserved patients across Texas
  • Manage chronic conditions acute concerns and routine follow-ups through telemedicine
  • Build trusted relationships with patients who have limited access to consistent medical care
  • Support and refine clinical programs focused on hypertension diabetes medication adherence and reducing preventable hospitalizations
  • Partner with care coordinators CHWs and RNs to close care gaps and ensure seamless follow-up
  • Provide clinical guidance during case reviews and escalations
  • Collaborate with product and operations teams to improve care delivery workflows
  • Join a high-energy mission-driven team reshaping Medicaid care delivery

What Youll Need

  • 2 years of clinical experience preferably serving medically or socially complex populations
  • Active Texas NP license
  • Bilingual - Spanish and English fluency required
  • Must be physically located in Travis County (AustinTX) with access to a quiet HIPAA-compliant internet-connected workspace
  • Experience supporting patients with chronic disease behavioral health needs SUD homelessness or Medicaid coverage
  • Comfort working within a multidisciplinary care model
  • Board certification (or eligibility) in Family Medicine
  • Comfortable delivering telemedicine and working across multiple digital platforms
  • Ability to care for patients across the lifespan (pediatrics to geriatrics)
  • Familiarity with FQHCs safety-net care or Medicaid populations preferred
  • Startup or fast-paced environment experience is a plus
