Case Manager, Welcome HOME Program
Principle Function: The Welcome HOME Case Manager will build relationships with young adults ages 18-24, by providing support and guidance to individuals who are going through challenges. The Case Manager will help clients develop their skills and confidence and assist them in reaching their personal goals. This position will assist the Welcome Home Supervisor in implementing and facilitating a client-driven calendar of events and activities. The Case Manager also receives intake calls, assesses clients' needs for service, and relays information about services provided by the Welcome HOME Drop-In Center.
Education and Experience: A bachelor’s degree in social work/psychology/or human services is required. Master’s degree in social work preferred. Two years of work experience in areas of job responsibilities preferred. Excellent relational, communication, assessment, problem solving, and organizational skills. Skilled in Trust Based Relational Intervention (TBRI®). First Aid and CPR training, reliable transportation, approved criminal background report.
