Care Team Assistant - Job Overview

Key Responsibilities
- Patient Communication: Use advanced communication tools to engage with patients, delivering lab results with care and clarity.
- Pre-Visit Planning: Utilize data analytics to optimize scheduling and gather critical insights in the electronic health record system (EHRS).
- Care Coordination: Facilitate patient transitions and ensure access to comprehensive services with innovative coordination techniques.
- Medication Management: Enhance efficiency in medication processes using tech-enabled systems for refills and prior-authorizations.
- Forms and Referrals: Use digital solutions to prepare forms and streamline referral processes efficiently.
- Safety and Compliance: Lead safety initiatives with dynamic training and tech-driven compliance strategies to maintain a secure environment.
Minimum Qualifications
- Education: A high-school diploma or equivalent is your gateway to innovation!
- Certifications: A Medical Assistant Certification or equivalent clinical experience is essential; BLS/CPR certification required for ensuring optimal healthcare delivery.
- Experience: Two years of experience in an ambulatory clinical setting is ideal for leveraging your expertise in dynamic environments!
- Skills: Mastery of EHRS, exceptional communication, strategic multitasking, and advanced organizational skills are key components of your tech-savvy toolkit.
- Extra Points: Bilingual proficiency in Spanish is a valuable asset in expanding our impact!
