Responsibilities
- Conduct PT evaluations for residents in a long-term care setting and develop individualized plans of care.
- Provide therapy interventions to improve mobility, strength, balance, and functional independence; modify plans based on patient progress.
- Document evaluations, treatments, progress notes, and discharge summaries in the medical record per regulatory and payer guidelines.
- Collaborate with nursing, therapy, and interdisciplinary team members to coordinate goals and discharge planning.
- Educate residents, families, and facility staff on exercises, safety, and home exercise programs to support ongoing progress.

Requirements
- Active Physical Therapist license in Texas
- Doctor of Physical Therapy (DPT) degree or eligible for licensure in Texas
- Relevant clinical experience; home health or long-term care experience preferred
- Ability to work in a skilled nursing facility setting and manage a caseload
- Willingness to travel for a temporary assignment and work daytime hours
- BLS/CPR certification preferred
